In connection with my voluntary involvement in the charitable activities undertaken for and in connection with the Baltimore Community ToolBank, a non-profit organization, I hereby agree for myself, my heirs, assigns, executors, and administrators to release and discharge the ToolBank, its officers and directors, employees, agents and volunteers from all claims, demands and actions for injuries sustained to my person and/or property as a result of my involvement in such activities. I agree to hold the ToolBank, its officers and directors, employees, agents and volunteers harmless from and cause of action, claim, or suit arising therefrom. I hereby attest that my attendance and involvement in such activities is voluntary, that I am practicing at my own risk, and that I have read the foregoing terms and conditions of this release. I also grant the ToolBank permission to use and distribute my image and voice for the purposes of media and marketing materials pertaining to the ToolBank without financial compensation.

Coronavirus Visitor/Volunteer Acknowledgment
The safety of our staff, volunteers, and the people in the communities we serve is our greatest priority. The Baltimore Community ToolBank acknowledges that while there have been significant improvements for prevention and treatment of severe illness, COVID-19 is still real and can present a threat. We ask that volunteers who have tested positive within the last five days, or are experiencing the following symptoms, regardless of vaccination status, please stay home.

Congestion or runny nose

While on-site, the Baltimore ToolBank will provide handwashing stations and hand sanitizer for all guests.
