This completed request form.
A copy of a photo ID of the person making the request (School picture ID, State ID, Driver's License, or Passport)
Attach any forms that BHS might need to verify on your behalf (SS benefit, Insurance discount Forms, Military, etc.)
If you are a Military Recruiter making a request; we must have a Transcript Consent on the student’s file.
Transcript Consent is available on the registrar's office site, and should be attached with this request.
Allow Up to 5 school days for processing.
