COVID-19 Liability Waiver

PLEASE READ:

Although the Province of Ontario is changing its COVID safety protocol, the Toronto Fashion Incubator will be maintaining its safety protocol by requiring proof of double or triple vaccination protocol. We are making this decision to keep everyone working at TFI as safe as possible.

Everyone entering the Toronto Fashion Incubator, including TFI staff, TFI members and their guests, current Residents, their team and their clients, MUST upload proof of double vaccination prior to arriving. 

Proof of your government-issued double COVID vaccination will be asked only once by completing this form and uploading proper documentation. This may change depending on government regulations such as if booster vaccines are required or if there are signs that a new wave of COVID threatens public health.

Only scheduled guests and visitors who have uploaded their vaccination form will be allowed into the building so please arrive alone and leave family and friends at home as there is no place to wait inside the building. For this reason, please arrive on time and not too early for your appointment.

Upon arrival, please buzz in. Your name is required prior to being admitted inside the building so that we can cross-reference your name to your proof of double vaccination form.

A medical mask that fully covers the nose and mouth must be worn at all times while inside the building.

SOME NEW SAFETY PROTOCOLS TO EXPECT

When you arrive at TFI for your appointment, please buzz in and follow the instructions given by our receptionist. The door will remain locked until it is safe for you to enter. If you have not submitted proof of your double or triple COVID vaccine, you will not be permitted to enter.

Upon entering, we will have you sanitize your hands and we will may ask some additional, recommended pre-screening questions and/or take your temperature. This information will be kept at TFI in the event Toronto Public Health requires it.

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that Toronto Public Health authorities still recommend practicing social distancing.
I further acknowledge that the Toronto Fashion Incubator has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that Toronto Fashion Incubator cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to staff, residents, clients and their families.

I am voluntarily entering the Toronto Fashion Incubator and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all of TFI's set protocols to reduce the spread while inside the premises. I agree that if I fail to comply with TFI's protocols that I will be asked to leave the premises immediately. I also agree that if I begin to feel unwell and/or show symptoms of illness I will inform TFI Admin staff and will leave the premises immediately.

I hereby release and agree to hold Toronto Fashion Incubator harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the establishment, or that may otherwise arise in any way in connection with any services received from Toronto Fashion Incubator.

I understand that this release discharges Toronto Fashion Incubator from any liability or claim that I, my heirs, or any personal representatives may have against the establishment with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Toronto Fashion Incubator. This liability waiver and release extends to the salon together with all owners, partners, and employees.

I acknowledge that by providing my personal information, TFI can use my information to contact health authorities in case of a suspected outbreak originating at or near TFI.
