COVID-19 Liability Release Waiver - 2020

**Signature Required Prior to Every Scheduled Appointment**
Due to the 2019-2020 outbreak of the novel Coronavirus (COVID-19), JTorry LLC / JTorry Makeup & Hair Artistry, LLC is taking extra precautions with the care of every client to include health history review and enhanced sanitation/disinfection procedures in accordance with the Texas Department of State Health Services guidance.

Symptoms of COVID-19 include:
- Fever (100 * or highter)
- Fatique
- Dry Cough
- Difficulty Breathing
I agree to the following: *

I understand the above symptoms and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above WITHIN THE LAST 14 DAYS. I affirm that I, as well as all household members, have not been diagnosed with COVID-19 WITHIN THE PAST 30 DAYS. I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 WITHIN THE PAST 30 DAYS. I affirm that I, as well as all household members, have not traveled outside of the country, or to any city considered to be a "hot spot" for COVID-19 infections WITHIN THE PAST 30 DAYS. I understand that out of respect and safety of Jenny and her Team member, I and my party will wear a mask and sanitized our hands before receiving services and when coming near the Team. Jenny & her team will advise when it's time to remove the mask. I understand that I will wash my hand or sanitize my hands before receiving service. I understand that my teamtpure will be taken. I understand if my temperature is high and show sign of sickness services will not be provided. I understand that Jenny Torry and JTorry LLC Team Employee, Contractor or Artist cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.
JTorry LLC / JTorry Makeup & Hair Artistry, LLC is following these enhanced procedures to prevent the spread of COVID-19:
- Each client required to wash hands upon arrival and before leaving
  Your artist will thoroughly clean hands and wear nitrile gloves during all services.
- Your Artist will wear a clean set of gloves, mask, face shield or goggles and an apron for each client
- All equipment including tools will be properly sanitized before and after use on client.
- All surfaces will be wiped thoroughly with hospital grade disinfectant before and after each client according to the manufacturer's directions
Additionally, to the above, we ask to limit the number of people in the room. We ask that there will be a space designated for the JTorry team, and to limit the number of people in the room to 3 people at a time. If one of the artists are not working on a client, we ask any additional people will be getting ready or mingling in another space. This will help reduce the potential exposure and will assist with social distancing.

By signing below, I agree to each statement above and release Jenny Torry and JTorry LLC / JTorry Makeup and Hair Artistry, LLC from any and all liability for the unintentional exposure or harm due to COVID-19.

JTorry LLC / JTorry Makeup and Hair Artistry, LLC. agrees to abide by these standards and affirms the same.
