A Holiday Card to the Ephrata Community

Please complete the following form and return it with a professional-quality photo of a family, couple, person, or office/business photo you would like to share. Please see below for photo specifications. (NO LOGOS for business submissions accepted. Must be a photo of you and/or you with office staff)

Sponsorship is $175.00 per banner.   

WE WILL ACCEPT TRADITIONAL ORDERS UNTIL OCTOBER 10, 2026.

Proceeds from the banner program will support the many activities sponsored by Mainspring of Ephrata.

If submitting via this form: please be sure to attach your photo in the next field. If you do not have a digital copy, you can submit online but please stop by the Mainspring office at the address below with your physical photo to have it scanned.

If you would rather send a check or do cash, please drop off or mail your photo, application & payment to:

8 a.m.-4:30 p.m. Mon-Fri, or put in an envelope and drop in the mail slot at the side door. (Parking Lot side door)

Photo Requirements: 

• Professional Quality Photos High Resolution (minimum 150 dpi at full photo size)
• Proper Attire
• Vertical Format, if possible
• Photos must be either a digital copy or scanned JPEG.

Physical Photos WILL NOT BE RETURNED. If you need your photo back, please stop by the Mainspring office with your photo to have it scanned.

By submitting this form, I hereby grant the use of the enclosed/emailed photo to the 2025 "Ephrata Holiday Banner Program" without payment or other consideration. I recognize that I have the legal right from the photographer to release this photo. I have secured permission from the person in the photo (if living) to display their image. All application information is accurate, to the best of my knowledge. Mainspring of Ephrata reserves the right to reject inappropriate photos and make all final banner decisions. I understand that Mainspring of Ephrata will not return any physical photographs. Submitted photographs should be copies or digitally submitted.
