Pledge to Confidentiality

One of the most harmful things that can happen in a healthcare organization is the misuse of confidential information. We want to make sure that you understand that we take confidentiality very seriously. Please review the information below and sign where indicated to acknowledge your agreement and understanding of the following:

As an employee, provider, vendor or volunteer of Reid Health, I understand that I will have access to very sensitive and confidential information about patients, employees or medical staff members as well as Reid Health’s business in general. I understand the vital importance of maintaining all such information, and any and all related discussions, in strict confidence. I understand the obligation to maintain our patients’ rights to privacy and confidentiality in all matters related to the patient’s personal life, medical care and patient care services.

I will*

only access, use, read or disclose information for which I have a business reason and am authorized to do so.

I will not*

obtain/access or seek to obtain/access confidential information which I do not need in order to carry out the duties and responsibilities of my position.communicate confidential information to any person who does not need it to carry out the duties and responsibilities of their position.assist others in obtaining confidential information not required to carry out the duties and responsibilities of their position.share personal sign-on identifications and/or passwords.will not leave a computer unattended without first signing off or applying a screensaver in accordance with the organization’s policy.discuss patients’ information, confidential human resources and personnel issues or the organization’s proprietary and/or confidential business in public areas such as the dining room, hallways, elevators or off the premises in the community.

I understand*

this agreement applies to all patients, employees, providers and volunteers and to any action which violates the spirit of the confidentiality pledge.breaches of confidentiality are matters of serious consequence that may harm patient care, slander a patient and his/her family, or lead patients, providers or others not to choose Reid for services.breaches of confidentiality regarding confidential human resources and personnel issues or the organization’s business in general and specific areas, cause a disruption of work and adversely affect morale.discussion, comments or inquiries about another specific or individual team member’s compensation (salaries and benefits, etc) are inappropriate and unacceptable.I have an affirmative duty and obligation to immediately report breaches of confidentiality to my supervisor, Human Resources or Reid’s privacy officer.that violations of any of these policies may result in my separation of employment, termination of participation in the volunteer program or other disciplinary action, without previous warning.should I separate employment with Reid Health, I remain obligated to the same confidentiality expectations and will not disclose any confidential information I gained while employed.
