Parents/guardians may request a fluid milk substitute be offered in place of regular milk when there is a medical or other dietary need. This request does not have to be signed by a medical authority. A milk substitute will not be offered or served unless we have this form on file for your student. The type of substitute is at the district's discretion.
IMPORTANT: if any other dietary restrictions need to be made, we must have a form from a medical authority to do so. Our preferred district form is also located on the FISD Child Nutrition website under "Special Dietary Needs".
