Payment Policy: Hopi AH does not offer billing or accept checks. Payment is due when services are rendered. An written estimate of fees will be prepared upon request. If you have provided credit card details for your account you agree to have your credit card processed, without signature, for any services provided or medications dispensed. If payment is not received or the bank fails to honor any credit card transaction deposited by Hopi AH, all service fees, including any filing, collection and/or attorney fees will be the responsibility of the client until account balance is settled.
Cancellation/No Show Policy: Due to high demand for appointments, when a client cancels without advance notice, it prevents another patient from being seen. For this reason, our 24 hour cancellation policy will be strictly enforced. A No Show/No Call appointment will automatically incur a missed appointment fee. Failure to cancel an appointment at least 24 hours in advance may result in a late cancellation fee. Repeated missed appointments will require pre-payment of the exam fee to schedule future appointments. Missed surgery appointments will result in a missed appointment fee and will require 50% down payment of the surgery estimate to reschedule.
Social Media/Photograph Policy: Hopi AH reserves the right to use any photographs taken in the hospital for social media and/or marketing purposes.
