Payment Policy: Hopi AH does not offer payment plans or accept checks. Payment is due when services are rendered. We accept Visa, Mastercard, Amex, Discover, and Cash. A written estimate of fees will be prepared upon request. A 50% deposit of estimated fees will be required on hospitalized patients.
Cancellation/No Show Policy: Due to high demand for appointments, when a client cancels without advance notice, it prevents another patient from being seen. For this reason, our 24 hour cancellation policy will be strictly enforced. A No Show/No Call appointment will automatically incur a $25 charge. Failure to cancel an appointment at least 24 hours in advance may result in a $25 late cancellation fee. Repeated missed appointments will require pre-payment of the exam fee to schedule future appointments. Missed surgery appointments will result in a non-refundable $75 fee and will require a 50% down payment of the surgery estimate to reschedule.
Social Media/Photograph Policy: Hopi AH reserves the right to use any photographs taken in the hospital for social media and/or marketing purposes.
