Hopi Animal Hospital Boarding Contract

ILLNESS EXPOSURE:

There is possible exposure to other animals and/or contagious diseases when you elect to board your pet. Through vaccination requirements and disinfecting protocols, we do our best to avoid this problem; however, illness does sometimes occur since animals can have a contagious virus without showing any clinical signs and therefore is unknown to us. For this reason, Hopi Animal Hospital will NOT be responsible for any illness your pet may contract and you will be responsible for the cost of treatment during or after your pet’s stay.

If your pet starts showing signs of illness while here, an exam may be done. If needed, we will provide immediate care. We will make every effort to contact you or the emergency contact at the numbers you provided.

*In the event of an emergency, we will take life saving measures while we attempt to reach you or your emergency contact. You will be responsible for fees.

IN THE EVENT YOUR PET REQUIRES MEDICAL ATTENTION, PLEASE CHOOSE ONE OF THE FOLLOWING:

Accommodations: Hopi Animal Hospital reserves the right to immediately change your dog’s accommodations if we believe it is necessary to protect the health and well-being of your dog, other dogs, or our staff.

Hours: Our drop off and pick up hours are Mon-Fri, 7:30am-5:30pm. This facility is not staffed with overnight care. Hours may change without notice. Please call for holiday hours.

Flea and Tick Prevention: We require all guests to be current on a flea and tick preventive. Flea and tick treatment provided by us and protects your pet for 30 days. If you do not have proof upon arrival, we will administer a flea and tick preventive. (fees apply)

Feeding: We provide Purina EN canine and Purina Pro Plan Adult feline. If your pet has special dietary needs, please provide the food or allow us to provide it at current charges. If you do not provide feeding instructions we will feed your pet as we see fit.

Group Playtime: We offer FREE group playtime. With your permission only and if available, your dog will be placed in a group that best matches their size and temperament. Please understand, while group play is always supervised, injuries can occur and Hopi Animal Hospital will not be held liable.

Baths: We offer a free exit bath for our canine guests. This is a drip dry bath only and may not include a brush out.

Vaccination Requirements: We require all boarding animals to be current on the following vaccinations: Dogs: DAPPV (or similar combo) Rabies and Bordetella. Canine Influenza is no longer required but is recommened. Cats: FVRCP, FELV and Rabies. If your pet is not current or you cannot provide proper documentation, we will vaccinate your pet at your expense. An exam is required for vaccinations. Home vaccinations will not be accepted.

Belongings: You are welcome to bring bedding, bones, chews, toys etc. Despite our best efforts, sometimes items get lost or destroyed. Hopi Animal Hospital is not responsible for items lost or destroyed.

Pictures: I agree to allow Hopi Animal Hospital, our representatives and employees to use his or her pet’s name and any images or likeness of his or her pet taken while the pet is staying at Hopi Animal Hospital, in any form or format, for use, at any time, in any media, marketing, advertising, illustration, trade or promotional materials.

Abandonment Clause: Your pet will be considered abandoned if you do not pick up within 30 days of pick up date and you have not contacted us to make other arrangements. Ownership of your pet will be relinquished to Hopi Animal Hospital and your account including all legal fees will be turned over to a collection agency.

Payment: Payment is due in full upon pick up, including any additional fees that may arise due to necessary medical treatment. Prices may change without notice. We do not accept checks.

With my signature hereunder, I authorize Hopi Animal Hospital and it’s designees to treat any medical condition that should arise during my pet’s stay. I understand that this facility is not staffed beyond regular office hours. I have read, understand, and agree to all terms on the boarding contract for Hopi Animal Hospital. Furthermore, I understand that I am electing to board my animal in an environment with other animals and will solely be responsible for any fees for medications and/or treatment if they should become ill during or after their stay.
