Custom Fit is now kindly requesting that all vendors who will be receiving training reimbursements submit a W-9 form annually. This helps us keep everything up to date and ensures smooth processing.

1. Name of entity/individual. An entry is required. (For a sole proprietor or disregarded entity, enter the owners name on line 1, and enter the business/disregarded entitiy's name on line 2).*
2. Business name/disregarded entity name, if different from above.
