INSTRUCTIONS:

DEPOSIT the amount of PHP1,200.00 TO IBP CEBU CITY ACCOUNT WITH BDO ACCOUNT NO. 002938003954.

The amount covers only for ONE (1) COPY of the following documents:
1. OBC CERTIFICATE OF MEMBERSHIP IN THE PHILIPPINE BAR with 2 Documentary Stamps. [For additional Copy/Copies, in addition to P1,200.00, the fee is P140.00/Copy];
2. OBC CERTIFICATE OF GOOD STANDING for LEGAL PURPOSE and APPLICATION/RENEWAL OF NOTARIAL COMMISSION USE ONLY with Documentary Stamp. [For additional Copy/copies, in addition to the P1,200.00, the fee is P140.00/copy. If you intend to request for OBC CERTIFICATE OF GOOD STANDING with documentary stamps for the following purposes, in addition to the P1,200.00, the fees are as follows:
a. PHP480.00/Copy - For Judicial and Bar Council Applications; and
b. PHP640.00/COPY – For Use Abroad.
3. BAR RATING WITH Documentary Stamp (Not required for Notarial Commission Applications but may be requested for job applications requiring Bar Ratings);
4. RTC - CEBU CITY EXECUTIVE JUDGE CLEARANCE (For filing of request for renewal of Notarial Commissions only. Notary public must followup the release of clearance because clearance is issued only upon complete submission of notarial reports for the year covered) with Documentary Stamp;
5. RTC – CEBU CITY/MANDAUE CITY/LAPU-LAPU CITY CLEARANCE with Documentary Stamp;
6. MTCC – CEBU CITY/MANDAUE CITY/LAPU-LAPU CITY CLEARANCE with Documentary Stamp; and
7. IBP-CCC CERTIFICATE OF MEMBERSHIP and IBP CERTIFICATE OF GOOD STANDING.

PICKUP/DELIVERY OPTIONS

AUTHORIZATION FOR PICK-UP: I hereby authorize the IBP-CCC to RELEASE the requested documents to my authorized representative , blanks , with ID Type and ID No. blank . Note: You will be notified when your documents are available for pickup. You may pick-up your documents at the IBP-CCC Office at IBP Building, Capitol Compound, Capitol, Cebu City during office hours.

PREFERRED DELIVERY ADDRESS

AUTHORIZATION FOR DELIVERY BY COURIER: I hereby authorize the IBP-CCC to deliver the requested documents to my preferred delivery address. I also undertake to pay the courier fee upon delivery of the documents.

In compliance with the requirements of the Data Privacy Act, I authorize the general use of information obtained from me in the course of this undertaking as may be deemed necessary by the Integrated Bar of the Philippines-Cebu City Chapter ("IBP-CCC"). The data, which includes my personal information may be collected, processed, stored, updated or disclosed by the IBP-CCC or continually be collected, stored, processed and/or shared for five (5) years from the conclusion of this undertaking with IBP-CCC or until the expiration of the retention limits set by applicable law, whichever comes later, for legitimate purposes and/or in compliance with any reportorial requirements.
