• Faith Formation Registration Form for classes at Blessed Sacrament

  • Parents, if you are new to the parish, you must first register the family at www.mybscc.org and click I'M NEW. You may then register your children for Faith Formation below. If you are already registered with the Parish Office, you may register up to 4 childres on this one form.

    Padres, si es nuevo en nuestra parroquia, deberá registrar su familia primero en www.mybscc.org y oprimir I'M NEW (soy nuevo). Luego podra registrar a su hijo(a) a la Formacion de Fe aqui abajo. Si ya esta usted registrado a nuestra parroquia, podra registrar hasta 4 ninos(as) en esta forma.

  • If your child is NOT BAPTIZED, please register for RCIA for Children at https://www.mybscc.org/rcia

  • You will be contacted ONLY if there is an issue with your registration.

    Names and Room numbers will be listed outside on the first day

  • Attention Year 2 Confirmation Students: You and your parent must bring the registration form and required documents to register IN PERSON at the Faith Formation Office located on the corner of 12th and DeSoto Streets on Mondays or Tuesdays during August between 10 a.m. – 3:00 p.m. Remember, parents, that you received the required documents from Sarita (BSCC) and Christine (STR) in May after class.  New families may contact Chris Stalnaker with any questions at cstalnaker@mybscc.org. Thank you for your cooperation.  

    Atención estudiantes de Confirmación de Año 2:** Usted y uno de sus padres deben traer el formulario de inscripción y los documentos requeridos para inscribirse **EN PERSONA** en la Oficina de Formación en la Fe, ubicada en la esquina de las calles 12th y DeSoto, los lunes o martes durante el mes de agosto, entre las 10:00 a. m. y las 3:00 p. m. Padres de familia, recuerden que recibieron los documentos requeridos de Sarita (BSCC) y Christine (STR) en mayo, al finalizar las clases.   • Photo Release Form / Divulgación de fotos

    Parent(s)/Legal Guardian(s): Your decision to enroll in the Blessed Sacrament Catholic Church Faith Formation Program provides your consent, authorization and permission for the Diocese of Orlando and Blessed Sacrament Catholic Church of Clermont, Florida, its agents, employees or duly authorized representative to take photographs, motion pictures, video or audio tapes of enrolled students. Enrollment also provides your consent for the publication, circulation and dissemination of said photographs, motion pictures, video or audio tapes or any duplication or facsimiles thereof for purposes it may deem proper, within Diocesan guidelines. In enrolling in this program, you also are relinquishing and giving to the Diocese of Orlando all right, title and interest in the finished pictures, negatives, reproductions or copies, and further waiving any and all rights to approve the use of such photographs, motion pictures, video or audio tapes and so release any and all claims of any nature whatsoever arisen for their use.

    Padre(s) tutor(es) legal(es): su decisión de inscribirse en el programa de formación en la fe de la iglesia católica de Blessed Sacrament proporciona su consentimiento, autorización y permiso para la Diócesis de Orlando y la Iglesia Católica Blessed Sacrament de Clermont, Florida, sus agentes, empleados o representante debidamente autorizados para tomar fotografías, películas, video o cintas de audio de los estudiantes inscritos. La inscripción también proporciona su consentimiento para la publicación, circulación y difusión de dichas fotografías, películas, cintas de audio o video o cualquier duplicación o fascímiles de los mismos para los fines que considere adecuados, dentro de las pautas diocesanas.
