1. Refunds are only given within the first 30 days of class. There are NO refunds if you chose the PAY IN FULL option. 

2. We use text message and email as correspondence as the main source of updates, events, details, closures etc. Please make sure you check your text messages from Legworks. All updates are posted on our socials first. fb page Legworks Dance Studio Ltd. and instagram @houseoflegworks. Be sure to follow both!

3. Costume deposits are $75 non-refundable. Due by November 1st. All auto-pay clients will be deducted for costume deposits in November, and balances in January in addition to tuition. 

4. Please contact us right away if your address, email, or phone number has changed. 

5. Accounts must be paid by the 10th of every month or we charge the card on file. A $15 late fee will be applied to your account if your card is declined. 

6. Ticket sales for our annual show are non refundable after purchase.

7. I understand if the account falls deliquent at any time (30 days or more, my child will not be permitted to take class if auto processing fails. 

8. Auto-Pay: If you choose to select the auto-pay option, your tuition will be deducted on the 1st of each month. During Costume deposit time (November 1st) we will also deduct a $75 deposit & Costume balances (January 3rd) are due as well with varying balances. Any unpaid tuition will be sent into collections and may affect your credit.

9. Attendance is crucial at Legworks. Please call to let us know if your child will be absent. Excessive absences may lead to dismissal. 

10. Drop off/pick up must be on time. This is important so traffic flows smoothly in our parking area and teachers stay on time for their classes. 

11. Please arrive 5 minutes before class so teachers can check you in. Parents can view classes from the waiting room area.

12.  There is a $35 return check fee for any insufficient funds.

NOTICE OF WITHDRAWAL: Notice of withdrawal from any or all Legworks Dance Studio classes must be submitted in writing to legworksinfo@yahoo.com or by phone: 

(631) 399-3930. This notification is required to properly adjust your account. If notice is not given your account will continue to be billed automatically throughout the dance season and you will be responsible for the charges. 

I have read the basic policies of Legworks Dance Studio Ltd. Please note these policies are strictly enforced. 

RELEASE OF LIABILITY:  I hereby acknowledge and understand that participating in on-premise classes offered by Legworks Dance Studio may require a degree of physical fitness, agility, stamina, and strength and that it may involve strenuous exercise. I agree to assume full responsibility for any risks, injuries, or damages which may incur as a result of participating in any class offered by Legworks Dance Studio. I hereby release and agree to hold harmless Legworks Dance Studio, its owners, operators, employees and instructors from any and all liability whatsoever for any and all damages, losses or injuries, including death, I may sustain to my person or property or both, including but not limited to any claims, demands, actions, causes of action, expenses, and costs, including attorney fees, which may arise out of, result from or in connection in any manner with the participation in classes taken at the Legworks Dance Studio.

MEDICAL EMERGENCIES:  As the legal parent or guardian, I give permission to Legworks Dance Studio, its owners, and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or conditions and/or declare the participant to be in good physical and mental health.

PHOTO/VIDEO RELEASE:  I give permission for Legworks Dance Studio to take photos or videos of me or my child while participating in Legworks Dance Studio activities for promotional purposes. 

MASKS: Are optional. 

As I or my child are enrolling to participate in in-studio classes at Legworks Dance Studio as of September 9th, 2024, by signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending in-studio classes at Legworks Dance Studio and that such exposure may lead to any or all COVID-19 related symptoms, effects, or outcomes. 

I hereby release, discharge, hold harmless and covenant not to sue Legworks Dance Studio for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Legworks Dance Studio or participation in any of their activities. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Legworks Dance Studio, it’s employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Legworks Dance Studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Legworks Dance Studio program.
