Please complete this final report form for the Conversation to Action Mini-Grant that you received. If you apply for another grant from the Community Foundation, a final report for all previously received grants must be on file. The project financial summary should include a list of revenue sources (received) with amounts for each and a list of the project budget and actual expenses by category. Also, please detail the items in the budget for which the Community Foundation grant was used. If the grant funds are not expended for their intended purpose within one year of the grant issuance date, they must be returned to the Community Foundation unless written authorization has been obtained from the Community Foundation prior to the deadline.
