I, the undersigned and/or parent/guardian, have carefully read and understand the information provided in the above PDF document. I, the undersigned and/or parent/guardian, understand that I will be informed of any Dance Program cancellations (due to low enrollment) and/or waiting list for future Dance Programs. The Registration Information Booklet PDF will be available for you to download in the email confirmation you will receive after submitting this form; it will be highlighted in blue.
Dance Waiver

I, the undersigned and/or parent/guardian, do hereby give permission for my daughter/son/ward/self to attend and participate in the programs offered by Atyeo’s Dance Studio. I hereby acknowledge that by attending, teaching and/or participating in the program(s), there is a possibility of physical illness or injury to my daughter/son/ward/self; and I do hereby for myself and all others who might have a similar claim waive, release, and forever discharge any and all rights and claims for damages, that may arise now and in the future, which my daughter/son/ward/self may sustain and suffer while attending and participating in the program(s) at Atyeo’s Dance Studio.
Property, Personal Belongings and Studio Uniform

I, the undersigned and/or parent/guardian, and my daughter/son/ward/self, will respect the property and its belongings. I, the undersigned and/or parent/guardian and my daughter/son/ward/self, will not park or drive on the grass, or park at the side of the building during the winter months due to the dangers of falling snow and ice. I, the undersigned and/or parent/guardian, will ensure that my daughter/son/ward/self is properly dressed and prepared for class according to the Studio Uniform Information given, including hair. I, the undersigned and/or parent/guardian, understand if my daughter/son/ward/self arrives without proper dance attire, according to our Studio Uniform Information, she/he/they/I will be asked to sit and watch. Outdoor street shoes will not be worn in the hallway, washrooms or studios. I, the undersigned and/or parent/guardian will ensure my daughter/son/ward/self, will practice safe and appropriate cell phone and camera use in and around the studio, and at all studio events. It is highly recommended that photos and videos are not taken in our change rooms and washroom.
Payment and Late Fees

I, the undersigned and/or parent/guardian, understand that payment is due on the 1st of October, November, December, January, February, March, April and May. 12:01am on the 2nd of the month is late. I, the undersigned and/or parent/guardian understand that a late fee of 5% PER DAY of my monthly payment will be charged to my account if payment is late. The September Monthly Payment is included when submitting this Registration Form. All dance accounts must be paid in full by May 1st.
Attendance

I, the undersigned and/or parent/guardian, understand that all classes begin and end at their scheduled time. I, the undersigned and/or parent/guardian, understand if my daughter/son/ward/self, miss the warm-up portion of the class, she/he/they/I will be asked to sit and watch. If my daughter/son/ward/self misses a class, it is our responsibility to find out what was missed. There are no rescheduled classes or refunds for classes that my daughter/son/ward/self, personally, does not attend. Attendance is crucial for progress and performance. If my daughter/son/ward/self misses more than four recreational classes throughout the year (or more than two competitive team classes/practices), their placement in the class, Competitive Team, Competition and/or Showcase, may be forfeited. I, the undersigned and/or parent/guardian, understand that if my daughter/son/ward/self misses any dress rehearsal(s) scheduled, she/he/they/I will not be allowed to dance in the Showcase.
Discard and Drop Out

I, the undersigned and/or parent/guardian, understand that Atyeo's Dance Studio reserves the right to cancel the enrollment of a child for the following reasons: Non-payment or excessive late payments; Disrespecting and/or the unwillingness to follow our Terms and Agreements; A child has special needs which we cannot adequately meet with our current staffing patterns; Physical and/or verbal abuse to staff or children, by a parent/guardian or child. I, the undersigned and/or parent/guardian, understand there will be no refunds or credits provided for programs dropped after 4 weeks.
Video and Social Media

I, the undersigned and/or parent/guardian, allow Atyeo’s Dance Studio to photograph and/or videotape my daughter/son/ward/self for advertising purposes in local newspapers, on social media, posters, in television appearances and/or in dance class. I, the undersigned and/or parent/guardian, understand the full name of my daughter/son/ward/self will not be published without my permission. I, the undersigned and/or parent/guardian, understand that costumes and/or digital download of performances will only be sent when dance account payments are up-to-date.
Class Cancellation - Weather

If it is necessary to cancel classes due to bad weather, such as snow and ice, there will be a post on our studio Facebook page after 2:30 pm. Please do not assume classes are cancelled because of public school closures, road closures, power outages, etc. as weather changes as the day progresses. You will not be contacted by a personal phone call. There will be NO reduction of monthly payments. We will try our best to reschedule classes that have been cancelled.
Class Cancellation - Situations Beyond Our Control
Behavior
Drug and/or Alcohol

This is a facility that cares for the well-being of all our dancers, staff and parents/guardians, and the environment that we provide. It is unacceptable to enter our facility under the influence of drugs and/or alcohol. Our expectations are that you and your family members are not under the influence of drugs and/or alcohol when you arrive on the property. Please arrange to have another adult/guardian drop-off/pick-up your dancer, if necessary. Otherwise, we accept your withdrawal from our dance studio. Also, discussions regarding how to buy, sell and/or the usage of drugs and/or alcohol are not permitted.
Profanity and Negativity
Impersonation

No parent/guardian/teacher and/or dancer may use the name Miss Tracy and/or Tracy Atyeo, the studio name Atyeo's Dance Studio, and/or the studio logo(s), without my, the director's/owner's, written consent. No parent/guardian/teacher and/or dancer may impersonate me/Miss Tracy/Tracy Atyeo and/or Atyeo's Dance Studio, to get access to online dancewear sites, open a social media and/or online account, register for dance competitions/workshops, etc. Please understand this is a chargeable offense.
Family Members and Friends

I, the undersigned and/or parent/guardian ensure that anyone responsible for dropping off and/or picking up my daughter/son/ward/self, at the studio and/or any studio events, will be made aware of all the studio's Terms and Agreements, Studio Standards, Schedules and Timings, Rules, Expectations, etc. I, the undersigned and/or parent/guardian, take full responsibility for anyone who is responsible for dropping off and/or picking up my daughter/son/ward/self.
Dance Uniforms

Dance Uniforms can be purchased through our online Dancewear Store (available on our website). If purchased before August 23, items will most likely be available by the first day of dance class (as long as item is not on back order). If you are unsure of the size, do your best according to the information and directions available on the site. There is no time for private fittings before class begins.
