    Use this form to determine eligibility for WIOA Services


    Under WIOA, who are Adults, Dislocated Workers and Displaced Homemakers?
    Adults are individuals age 18 and older who are unemployed at time of application OR who are under-employed OR whose family meets adult low income guidelines (see Chart at bottom of page).

    Dislocated Workers are individuals with significant attachment to the workforce but have lost their employment or income from employment for one or more of the following circumstances:

    • Has been terminated through no fault of their own or laid off, or received a notice of no fault termination or layoff from employment and is eligible for or has exhausted U. I. or is not eligible for U. I. due to insufficient earnings or having performed work for a non-covered employer, but has been employed for duration to show attachment to the workforce (6 months) and is unlikely to return previous occupation without additional services or training


    • Has been terminated through no fault of their own (determined by UI review) or laid off, or received a notice of no fault termination or layoff from employment as a result of any permanent closure of or any substantial layoff at a plant, facility or enterprise.


    • Is employed at a facility at which the employer has made a general announcement that such facility will close within 180 days


    • Previously self-employed and unemployed due to natural disaster or general economic conditions


    • After layoff, have become reemployed in an income maintenance position and meets the definition of a currently employed dislocated worker (underemployed, see below)


    • Individuals who have been separated for cause and/or those who voluntarily left who are also eligible for UI. 


    • Being discharged (under honorable circumstances) either voluntarily or involuntarily from the military and unlikely to return to a previous industry or occupation. TEN 22-04


    Currently employed dislocated workers may be determined eligible for services if the customer’s current employment meets all of the following conditions:

    The individual is currently making less than 85% of former wages, and/or is in employment that uses significantly less skills or abilities than the job of dislocation and is not commensurate with the individual’s demonstrated level of educational attainment, and the individual’s current employment and job of dislocation is not in a field or occupation that is generally known for short term or contract work. In these instances, customers may be assessed for program eligibility at the time they are no longer working.

    Displaced Homemakers (male or female)

    • An individual who has been providing unpaid services to family members in the home and who


    • Has been dependent on the income of another family member but is no longer supported by that income and is unemployed or underemployed and is experiencing difficulty in obtaining or upgrading employment (may include spouses of dislocated workers)

    For Currently employed dislocated workers: Eligibility documentation may include tax return information (up to 2 years) validating an income decrease. This will not be required if dislocation & WIOA application occur within the same year.

    1.     Go to https://atlworks.org/ 
    a. Click on "Help For Job Seekers", then
    b. Scroll down to "Ready to get started" and
    c. Click on "GET STARTED". 
    d. Enter the information in the form, "Sign Up to Get Started", and
    e. click on "SUBMIT".

    2. Click on www.worksourceportal.com 
    a. Click on “no registered yet” next to the yellow sigh in button
    b. Scroll down to “individual” and click on that
    c. Fill out the details to create your account here

    IMPORTANT: Remember to have one (1) document ready in each category that applies to you below:

    • Proof of your Date of Birth (Birth Certificate, Passport, GA ID or GA Driver’s License)
    • Proof of your Social Security Number
    • Proof of Residency (Utility Bill, Lease, Notarized Statement)
    • Proof of your Citizenship or Authorization to Work in the U.S.
    • DD214 (Veterans Only)
    • Proof of School Status (Youth Only)
    • Proof of Income (Food Stamp Letter, TANF, SSI, Recent Pay Stub) or Unemployment status


    3. After receiving a response email, attend virtual Orientation.

    4. Upload the required documentation for Eligibility.

    IMPORTANT: Submit your documents within 30 days of attending an eligibility (virtual) orientation / information session. [The eligibility process can take up to 30 business days to complete if you dont have everything you need, beginning on the date you submit a completed packet.]

    IMPORTANT: If your packet is found to be incomplete during your eligibility appointment, it will be returned to you with all of your documents. You will have to make the necessary corrections and obtain any missing documentation and resubmit the complete packet. Note that the 30 business day window for processing starts when you submit a COMPLETED packet.

    Once your packet is approved you will receive an email indicating you are eligible for participation in the WIOA program.

    Within five (5) business days your assigned Career Planner will contact you for a first appointment.

    Authorization to Pay Charges______________________________________________________

    I do here by authorize S.A.H.P Training to use my financial assistance awards (including the Pell Grant) to pay any educational expenses that I incur at S.A.H.P Training. These charges will include, but are not limited to, tuition and fees, books, exams and supplies.     This program is an intense 16 hour training program broken down into two 8 hours days.  This program will concentrate on all 3 categories from service, hospitality and retail and their fundamentals.  We will go over proper service, etiquette and techniques in all three.  We will also go over all of the do’s and do not’s of the industry.  More importantly we will go over proper verbiage and communication techniques.  Furthermore, This class will be very hands on and interactive with mock services and scenarios. This intense two day training will prepare these individuals for a proper and well paying job in the service, hospitality and retail industries.

     

    YOUTH EXPRESS TRAINING AND JOB PLACEMENT PROGRAM
    (for youth ages 18 – 24)
     
    Program information:
    Duration: (16 hours)
    Days: (2) Sat – Sun
    Hours per day: (8)

    Program qualifications:

    After completing the program:
    After you complete this program and pass our exam, we will assist you with job placement at one of the many locations that we work with.

  • Statement of Responsibility for Financial Obligations
    For enrollment during the training, I promise to pay SAHP TRAINING my total financial obligation (tuition, fees and Bookstore charges) upon receipt of my financial award(s) or student loan(s).

    I understand and acknowledge that in the event that the anticipated fund(s)are denied or are otherwise not forthcoming, or in the event that I stop attending SAHP TRAINING for any reason, my total financial obligation shall become due and payable immediately.

    I understand and acknowledge that failure to fully satisfy my total financial obligation may result in collection and/or legal action brought against me by SAHP TRAINING.

    Further, I understand and acknowledge that I am fully obligated and responsible for any and all charges associated with such collection and/or legal action.
