Former Student Verification
I affirm that I am a former student of Louisville Beauty Academy requesting either an official or unofficial transcript.

Compliance with Kentucky Law: Louisville Beauty Academy is fully compliant with Kentucky State law (201 KAR 12:082).
The Academy is required to maintain student records for a maximum of five (5) years from the last date of attendance.
After five years, all student records and hours are legally void under state regulation. Louisville Beauty Academy cannot issue or certify transcripts for expired records.

Transcript Validity and Use
Any transcript issued by Louisville Beauty Academy is valid only within the five (5) year period from the student’s enrollment/start date.
I understand and agree that I may not use, present, or represent any transcript for credit, licensing, or professional purposes if it is older than five (5) years from my enrollment/start date.

Liability Disclaimer
Misuse, misrepresentation, or attempted use of an expired transcript is solely the responsibility of the student.
Any such misuse may constitute fraudulent activity and can be reported to the Kentucky State Board of Cosmetology or other appropriate authorities.
The student assumes 100% liability for any damages, claims, or consequences arising from misuse, including but not limited to:
Damage to the reputation and goodwill of Louisville Beauty Academy,
Regulatory or legal action,
All costs, attorney fees, and liabilities incurred by Louisville Beauty Academy in defending its compliance and reputation.

Financial Obligation
Transcripts will only be released if all financial obligations (tuition, fees, etc.) have been satisfied in full.
Processing Fee
$25 for a digital copy (sent by email).
$50 for a printed and mailed copy.
Payment is due at the time of request.
