1. MISSION/ARTISTIC VISION - Please provide your mission statement, if you have one. If not, how would you briefly describe your artistic philosophy or intentions in presenting, performing, or composing (as applicable)?
2. HISTORY/BACKGROUND - Please give a brief history of your ensemble or organization or, if an individual, your work and performance experience in your music field.
3. ADMINISTRATIVE - How do you conduct your business? Who makes the artistic, organizational, and business decisions? How do you divide money earned?
4. FINANCIAL - Do you currently have a Fictitious Business Name? If so, please list it below. Do you have a bank account (aside from your personal banking account) using that name?
5. PRIOR FISCAL SPONSORSHIP - Have you ever applied for and received fiscal sponsorship from another organization? If so, please list the organizations and type of project that was sponsored.
6. FUNDRAISING - One of the primary functions of Fiscal Sponsorship is to enable you to solicit tax-deductible donations and apply for grants from certain foundations. Have you done either of these activities in the past? If so, please describe.

The InterMusic SF Fiscal Sponsorship Program accepts ensembles, individuals or organizations that can articulate a project that is in conformance with its charitable purposes. The InterMusic SF Mission Statement:

Our mission is to support the professional and artistic growth of San Francisco Bay Area musicians whose focus is the small ensemble. Through programs that include fiscal sponsorship, grant-making, career development, and curated performance partnerships, we strengthen the careers of local artists who contribute to a vibrant and diverse music community.

We envision a thriving musical climate for San Francisco Bay Area musicians with sustainable careers who are celebrated by engaged and dedicated audiences. We value small ensemble music-making for its intimacy, immediacy, and champion its many genres by serving those who create and share it.
Why are you applying for the program at this time?

Your ‘project’ may be as general as getting a new ensemble up and running, learning what it will take to fundraise and preparing to do so. Below are some examples of more specific projects that could benefit from fiscal sponsorship:

• Self-presenting a series of local concerts for your ensemble
• Raising money to pay your ensemble to rehearse your compositions
• Applying for a grant to present a concert of new works by a local composer
• Producing a fundraising event to pay for a CD release
• Applying for a travel grant to perform on a festival you’ve been invited to
• Seeking funding for a series of outreach performances
• Asking for donations to commission a composer
• Mailing an appeal letter to 250 supporters to pay for expenses related to the launch of a concert series

This description should demonstrate your ability to carry out the project and your interest to raise funds for it. Some items you could address as applicable:

• What are the goals of your project?
• Does your project target a particular audience?
• How will your project impact the community?
• Does your project emphasize a particular repertoire or genre?
• What is the timeline? Will this be a one-time project, or ongoing? If one-time, will you want your fiscal sponsorship status to continue past the project’s completion?
• Are any dates, venues, collaborators, etc. confirmed at this point?
• To the best of your current knowledge, what is the budget of the project?
• What is your fundraising strategy?
