  • Medical Information

  • Resilience Charter School and/or the sponsoring school district may release and exchange my child’s confidential information to agencies of the State of Florida which would allow the school and/or district to verify Medicaid eligibility, bill Medicaid for reimbursable Certified School Match services referenced on my child’s individual educational plan (IEP) and receive Medicaid reimbursement for Exceptional Student Education (ESE) services it provides to my child while at school. I understand that my child will continue to receive services referenced on his/her IEP whether or not I give consent.

  • I understand that I must disclose to Resilience Charter School staff immediately if my child or anyone in our household, or anyone that we have been in close contact with, has tested positive for COVID-19. Any student with significant exposure to a positive case of COVID-19 will need to self-isolate for at least 14 days (schoolwork may continue from home if well enough). Any student returning to the school building after isolation will need written medical proof of a negative COVID-19 test to rejoin our community.

  • ACADEMIC INTERVENTION POLICY

As it is the primary intention of the school to provide the necessary support for each student to grow academically and achieve substantial learning gains every year, Resilience may require tutoring for students who have evidence of need for more academic support. This determination may be made upon evaluation of standardized test scores, school grades, classroom engagement & participation, or other formal and informal assessments. If a student and family are informed that tutoring or other interventions will be required in order to meet the student’s needs for remediation, but the student does not participate in the interventions required, Resilience reserves the right to recommend placement in another school. These decisions will be made as a team with the family and school administration and teachers, with Resilience administration holding the authority to make a final determination as to the student’s eligibility to continue attending the school.
