    I use gloves that are made out of Latex or PVC plastic.

    A NON REFUNDABLE BOOKING FEE/DEPOSIT IS REQUIRED AS WELL AS THIS SIGNED AGREEMENT IN ORDER TO SECURE YOUR DATE LISTED ABOVE. THE BOOKING FEE/DEPOSIT WILL BE DEDUCTED FROM THE FINAL BALANCE. THE REMAINING BALANCE IS DUE A WEEK BEFORE THE EVENT. THE CLIENT IS RESPONSIBLE TO PAY BLUE SAPPHIRE MAKEUP FOR ALL SERVICES PROVIDED


    THE CLIENT WILL LOSE THE BOOKING FEE WHEN CANCELING THE MAKEUP SERVICES AND IS RESPONSIBLE FOR ANY OTHER FEES, SUCH AS TRAVELING EXPENSES, ROOM, BOARD, PARKING AND TOLLS FEES.


    THE CLIENT MAY ADD MORE GUESTS TO RECEIVE SERVICES THE DAY OF THE EVENT, DEPENDING ON THE ARTIST SCHEDULE AND TIMELINE GIVEN TO HIM.


    THE CLIENT CAN SUBTRACT ANY GUEST FROM MAKEUP SERVICES, 2 WEEKS PRIOR TO THE EVENT. OTHERWISE, 30% OF THE RATE QUOTED PER PERSON WILL BE CHARGED PER PERSON.


    A $20 FEE WILL BE ADDED TO THE BILL FOR EVERY 1O MINUTES THE CLIENT OR GUEST IS LATE PAST THE AGREED TIME FOR THEIR SCHEDULED APPOINTMENT.


    IT IS THE CLIENT AND GUESTS TO DISCLOSE ANY ALLERGIES, SKIN REACTIONS, OR ANY SKIN ISSUES THEY HAVE AND MAY HAVE WITH CERTAIN TYPES OF COSMETIC PRODUCTS, INGREDIENTS OR SKINCARE PRODUCTS AND OR EXPERIENCING ANY COVID-19 SYMPTOMS AND ANY ILLNESSES.


    THE CLIENT AND PEOPLE RECEIVING SERVICES ACKNOWLEDGE THAT THEY NEED TO COMPLY WITH ALL SET PROCEDURES TO REDUCE SPREAD WHILE ATTENDING THEIR APPOINTMENT. THEY'VE ACKNOWLEDGED THEY COULD BECOME EXPOSED TO COVID19 FROM OMISSIONS, NEGLIGENCE ACTIONS OF THEMSELVES AND OTHERS, BUT NOT LIMITED TO SALON STAFF, OTHE SALON CLIENTS, AND THEIR FAMILIES.


    THE CLIENT AND GUESTS AGREED TO HOLD HARMLESS JOHAN GRIMALDI AND BLUE SAPPHIRE MAKEUP AND ANY OF THEIR EMPLOYEES AND STAFF FOR ANY LIABILITY IF ANY.


    BY SIGNING THE AGREEMENT YOU ACCEPT ALL THE TERMS AND CONDITIONS STATED ABOVE.
