注意事項：
1. 辦公室會發出最後確認電郵予成功遞交申請之組長或事工領袖。
2. 借用物品及鑰匙必須歸還原處，廚具及爐具使用後必須清洗乾淨。
3. 離開前請必須關好門窗、冷氣、房燈及電視/電腦/音響器材。
4. 若你在過去一週身體感到不適，如發燒或體溫高於100℉、咳嗽、流鼻水等徵狀，請即停止參加任何教會實體活動及崇拜，並及早求醫。
Notice:
1. CBMC Office will send a final confirmation email to the leader who successfully submitted the application.
2. Borrowed items and keys must be returned to their original places, and kitchen utensils and stoves must be cleaned after use.
3. Please close doors, windows, air conditioners, room lights, and TV/computer/audio equipment before leaving.
4. If you have felt unwell in the past week, such as fever or body temperature above 100℉, cough, runny nose, etc., please stop participating in any physical church activities and worship services immediately, and seek medical advice as soon as possible.
申請借用教會房間者必須為小組組長、團契導師或事工領袖，並只供教會活動之用。Applicants who apply to make reservations for church rooms must be small group leaders, coach/head coaches, or ministry leaders, and it is only for church activities. *
