Guest Form Instructions (Prep)
NOTE: This form is for guest appearing on KRQE This Morning between 7:00 am - 9:00 am. If you are appearing on *New Mexico Living* between 9:00 am - 10:00 am, please use the Client Booking Form.1. Form must be completed no later than 3 days prior to interview date or your interview may be cancelled.2. Please allow yourself at least 10min to fill out form completely. This form cannot be saved for edits.3. Fields with an asterisk * are required4. The Primary Guest will be the primary contact for the interview and confirmation.5. Please provide a phone number for each guest.6. Have all information ready before you fill out form such as:- Guest names, phone number, email address, title- Website and all social account URLs- File uploads such as photos, logos, video, audio- Your topic and talking points- List of props- Event details
Release Acknowledgment
To the extent I perform any music, read any literary pieces and/or submit any photographs, audio, video or other artistic works to the Station, whether original or otherwise, I hereby represent and warrant that I own, control or have obtained all rights (including all copyrights) in and to all such materials (“Materials”) and I hereby grant the Station, without charge, the rights necessary to use all such materials in connection with the Program in any and all media now known or hereafter devised for any purpose throughout the universe in perpetuity.
