Placing your gift basket order with Baskets Extreme is quick and easy! You can be assured that your order will be processed promptly and that your basket will be hand crafted with special care. Fill out this order form in its entirety and make sure to include as much information as possible so we can bring your vision to life. Please allow 1 week for fulfillment. We will do our absolute best to have your order completed by your desired date. Please allow up to 72 hours to fulfill your order. If you do not require delivery, you will receive a text or e-mail with the pickup address once your basket is complete.

*A local delivery fee of $20 will be added to each order, for each address within a 10-mile radius of our service location. For deliveries outside our service location area, the delivery fee may vary. Gift basket orders must be received before noon to assure same-day delivery. Orders received after that time will be delivered the following day. We will do our best to accommodate deliveries at specific times of day, but we cannot guarantee it.*

Payment Information

Once we have received your order form we will send you a payment request from the payment method you indicate below. Cash payments are accepted but do REQUIRE a $25 deposit via (PayPal basketsextreme@gmail.com) All deposits will be applied towards the total purchase. Balance is payable at time of pick-up/delivery. Please note that once your deposit has been received it secures your basket order. If remaining balance is NOT paid within 15 days - your order will be cancelled. DEPOSITS are NON-REFUNDABLE.
