  • Welcome to the DMK Online Order Form. We have implemented this special order form to assist you in receiving your supplies of DMK product during the COVID-19 period. Please use your mouse to click on the form and then use your TAB key to move to each input field and update as required. To move back to a previous field use Shift+TAB. You can also use your mouse to navigate through input fields.

    You can track the value of your order as you go, with the live total displayed at the bottom of each section. The total value of your order will change as you alter the quantities of each item ordered. All prices shown are GST inclusive and shipping is FREE.

     

    To get started please click on the form and tab to the "Your Name" field and add all the information in the boxes following. All this information is mandatory. 

  • Please note payment is required to be made before you submit this order. Once you have entered the items you wish to order you can enter your credit card details. These details will only be used to process your order and will not be held on file thereafter.

      To send your order to DMK please firstly confirm you have had an in-clinic skin assessment at a DMK Clinic by clicking "YES" below. If you click "YES" you will see the SUBMIT button and you can click it to submit your order. If you answer "NO" to the acknowledgement below you will not be able to submit your order.
