3-month Coaching Program Agreement

HerEquity Coaching Agreement - 3 month commitment

Please take a minute to read what is expected during the 3-month coaching package. You must agree to each expectation. If you have any questions, don't hesitate to call or email HerEquity LLC.

I promise to coach you with sound financial advice. My sole purpose is in helping you, the client.

My promise to honor your complete privacy. We will not share what we discuss with any other person without your specific and written permission first.

My communication is crucial to the success of our meetings. I will be truthful and transparent with my thoughts, ideas, dreams, goals, and challenges.

Your Commitment to HerEquity LLC...

I will show up to each appointment. “Showing up” involves being prepared with an updated budget and completed homework or prep work. It means having an open-mind, willingness to learn and positive & honest communication with my coach.

My communication is crucial to the success of our meetings. I will be truthful and transparent with my thoughts, ideas, dreams, goals, and challenges.

I will commit time each week to better managing my money. I will conquer homework assignments, projects, tasks or other work outside of meetings.

Payment and Scheduling

I agree to pay HerEquity LLC for the 3-month Coaching Program. The total price is $1,500 with a payment option of $250/bi-weekly over the course of three months, following the date of the Self Discovery session (initial payment of $300). There will be a $50 discount for committing to the 3-month Coaching Program.

I understand 3-month Coaching Program includes the following: my action plan created for me ahead of time, (1) Self Discovery Session and (5) One-on-One Coaching Sessions over the following 3 months, unlimited phone calls and emails.

While there will be prep work up front, my program begins the date of my initial session and will continue for 3 months (5 additional sessions) from the initial session

I understand that scheduling all the sessions during my program are ultimately my responsibility. I have the option of pre-scheduling all of these, scheduling them online, calling or emailing the office to get scheduled or using my time with my coach to schedule them. Any unused coaching time or sessions not used within the allotted time frame will be potentially forfeited, at the discretion of HerEquity LLC.

I understand that refunds are not offered on coaching packages. Should I decide to end my package early or terminate my package with HerEquity LLC for any reason, unused, paid coaching time has no cash value and will not be refunded.

I understand the cancellation & reschedule policy: I will not reschedule more than once per session unless due to extenuating circumstances. I understand that it is to my benefit to follow the program set forth by HerEquity in order to reach my goals and milestones.

I understand I have options such as doing the meeting over phone or web conference, which is often times better than cancelling or skipping the appointment altogether

Responsibility

I am solely responsible for my actions, choices, and decisions.California is the governing state for this agreement. A party's failure or neglect to enforce any of its rights under this agreement will not be deemed to be a waiver of that or any other of its rights.

I hereby release, waive, acquit and forever discharge Tina Pham, HerEquity LLC, their agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit, action, demand or right to compensation for damages I may claim to have or that I may have arising out of actions, omissions, or commissions taken by myself or by my coach as a result of advice given by Tina Pham or otherwise resulting from the coaching relationship contemplated hereunder. I further declare and represent that no promise, inducement or agreement not herein expressed has been made to me to enter into this release. The release made pursuant to this paragraph shall bind my heirs, executors, personal representatives, successors, assigns and agents.
