  • Eliminating Barriers to Learning Request Form for HDSB Students

    A request for emergency assistance through HLF's Eliminating Barriers programs can support a student’s basic personal needs, contribute to helping this student come to or stay in school, or help them participate in a school activity they cannot afford, such as a team, band, or field trip.

    This school year, until December 31st, HLF will support each student up to $250 for the same types of emergency needs we have previously supported. A student can receive support only once this school year. In January, HLF will evaluate how successful our Fall fundraising efforts have been. We will then determine if we will be able to maintain support at $250 per student or another amount. 

    Please use 1 form per household per school. Up to 4 students per household can be listed on this form. If there are multiple students from different households, please use a separate form for each family. If there are students from different schools in the same household, please ask the other school to complete a separate request.

    Eliminating Barriers funding is available only for students registered to attend HDSB schools and does not include support for children who are not in school or in another school board.

    If you are submitting for a reimbursement please ensure you have both a completed cheque requisition and receipts for the request.

    There are some items that HLF cannot fund through Eliminating Barriers, such as prescriptions, medical appointments, graduation expenses, therapy, presents, holiday celebrations or non-school courses.

    All funds for Eliminating Barriers come from donations from individuals, families, and organizations in Halton Region. HLF is grateful for the many HDSB staff who are payroll donors.

     

  • To ensure you receive support for the student as quickly as possible and to help reduce contact, we encourage ordering virtual cards. A virtual card will be sent to you as a pdf, which can either be: (a) printed for the student/family and provided to them in person; or (b) emailed or texted to the family. The family or student can print out or show the .pdf on their phone at the store checkout. Virtual cards usually take 3 school days to arrive.

    If you select the physical card option, a team member at HLF will contact you about a location for picking up the cards.

    Walmart cards cannot be requested for transportation, field trips, teams, or post-secondary applications.

    We cannot process a request that includes both cards and a reimbursement amount.

  • If you chose the reimbursement option, HLF will reimburse the school, not the household or requester directly.

    Please upload both a signed HLF cheque requisition form and accompanying receipts in the box below. The request for reimbursement is to be net of any HST rebate made on purchases.

    Both a cheque requisition and proof of payment are needed to process a reimbursement.
