Use this form to request one of the following:
- Option A
  A sick tray package that can be picked up by a friend.
  (Generally Sick or in isolation)
- Option B
  A packaged meal ordered from the Eatery Menu to be picked up by a friend.
  (Generally Sick or in Isolation)
- Option C
  A sick tray package or meal ordered from the Eatery Menu that is delivered to your on campus residence
  (only available for students in confirmed isolation/quarantine housing)

Sick Tray Packages vare available for students who are confined to their rooms and include the following:
- Vegetable Ramen
- Broth soup cup
- Saltine Crackers
- Applesauce cup
- Jello cup
- Bottled Water
- Orange Juice
- 1 cutlery kit
Weekday Meals:

Orders must be submitted by the following times:

Breakfast - by 8:15am

Lunch - by 11:15am

Dinner - by 4:15pm

Meals will be delivered / picked up between the following times:

Breakfast 8:30am - 9:30am

Lunch 11:30am - 12:30pm

Dinner 5:00pm - 6:00pm
