Commissions Transfer Request Form

DECLARATION

1. I acknowledge Young Living Philippines to perform my Commission Transfer Request.
2. I acknowledge that I am the account holder for the account that I am requesting the commissions transfer for.
3. I acknowledge that TIN or Tax ID is a requirement for the release of commissions.
4. I acknowledge that all monthly commissions with a net amount below PHP 1,280 will go to my account credits and monthly commissions with a net amount above PHP 1,280 could be transferred via Direct Deposit to my bank account.
5. I acknowledge that the following bank account details are accurate and correct:

that the bank is a PESONet member bank;
that the bank SWIFT Code in my VO is correct;
that the bank account is a Philippine Peso currency account;
that the bank account number in my VO is complete (including leading zeroes that comes before the first non-zero digit, if any); and
that the bank account holder name matches my Young Living Account membership name.

6. I acknowledge that I am the account holder for the bank informations saved in my Virtual Office account.
7. I acknowledge that for any commission transfer requests (for either Account Credits or Direct Deposits) it may take up to 10-15 business days.
