  • NYI/Youth Ministry Consent Form 2020-21

    Please complete this form if you would like your child to participate in the online (Zoom) and face-to-face NYI sessions (we will let you know when face-to-face sessions are able to start again safely).
  • We will use this information for the purposes detailed below and as instructed in your consent, and in keeping with our church's Privacy Notice.

    This consent applies for up to 1 year when the form is submitted for the activities detailed but you can contact us at any time if you wish to change or withdraw any of the details of consent given here. We will circulate additional consent forms for trips, youth camp or District events.

    • Personal and contact details  
    • We would like to communicate with parents/guardians via email and WhatsApp to let you know about meeting dates and activities. From time-to-time we may send NYI information or resources to your postal address.

    • NYI live Zoom group(s)

      NYI is currently meeting live via Zoom for online discussion and games, including break-out rooms. We use our Longsight Community Church of the Nazarene Zoom account. These sessions will not be recorded or made public, we will also prevent the private chat mode from being used. Although we will advise against it, be mindful that this does not stop participants from taking screen shots and sharing on their personal social media networks. Sessions will be led by at least two adults from our church who hold a current DBS.
    • For the group to work well we need consent for your child to participate in a way that we can see and hear them on Zoom. Sometimes we'll be playing games which only work if people can be seen and heard. This also helps us to know who is online with the group.

    • NYI face-to-face group(s) activities

      Depending on Government advice and regulations regarding Covid-19 we hope to be able to meet face-to-face for some discussion and non-contact activities (indoors and outdoors) from October 2020. We will review health and safety guidelines and our risk assessment(s) to discern with you and your child(ren) when and how we can meet safely. We will be sending more details of face-to-face meetings once these have been decided and appropriate safety measures have been taken. We will send separate consent forms for any activities off-site. Feel free to contact one of the Pastoral Team if you would like a copy of the current risk assessment(s) for our buildings.

      It's good to occasionally share with our church community and District what our group has been doing but, in keeping with our church's GDPR policy, we need your consent for any images or publicity where your child can be recognised or is named. We would like to celebrate and publicise group activities by featuring them in Longsight Community Church of the Nazarene communications materials, including printed publications or digital media, including Longsight Community Church of the Nazarene website and social media (Facebook & Twitter) which are also in the public domain. From time-to-time we may share these with our church District and their social media. Whenever possible, our usual practice is to try and gain additional, informal consent from the young people to make sure that they are happy with individual images that are used - we know that not everyone likes having their picture taken:-)
    • Finally...  
    • We don't expect you to sit with your child during the live Zoom sessions but we recommend that you discern whether someone needs to be available in case your child needs help with Zoom or any instructions given. We also recognise that as we connect online it's tricker to respond to a young person if they are upset. We'll try to follow-up as best we can but please do let one of the session leaders know if we can help in some way. 

      Also, please let us know if your child(ren) may have difficulties accessing internet data or devices to participate. We can help!
