Games for Good Citizens Class Registration Form
Games for Good Citizens classes are for dogs one year of age or older.

They will meet for five weeks, with sessions at:
- Kenwood Animal Hospital, 5439 Butler Road, Bethesda, Md.
Cost is $395 per client/dog pair.

Class size is limited, in order to best ensure safe social distancing practices and to provide personal attention for you and your dog.

No make-up classes are offered, and refunds cannot be given. Please enroll only if you can commit to class. Your spot in class will be reserved once payment is received.

As much as we urge each family member to train with your dog, only one adult member of the family can accompany your dog to class.

Requirements:
- The facility requires that we have current vaccination records in hand before the start of the first class. Please email that documentation to doglatinbeth@gmail.com.
- The dog must be at least 18 months old.
- The dog has prior training experience.
- The dog has a confident, friendly temperament with a curious or indifferent attitude toward other dogs.
- No prong collars, choke chains, or retractable leashes are allowed. Body harnesses are preferred.

Policies and conditions
Classes are taught by Dog Latin Dog Training, LLC and subcontractors. Classes are limited to no more than four client/dog pairs. Enrollment includes three 45-minute group classes.

Dog Latin Dog Training and subcontractors may, at their discretion, refuse entry to any dog that is not healthy, is aggressive, or is not deemed suitable for the services provided.

Owner is responsible for cleaning up any urine or excrement from their dog.

Unless I deny consent, Dog Latin Dog Training may use still or video images of my dog on social media. Dog Latin Dog Training will only use the dog’s first name and will neither mention the family surname nor its geographical location. This consent does not apply to paid tadvertising, for which Dog Latin Dog Training would execute a separate agreement and release.

I understand that attendance at a dog training class is not without risk to myself and my dog.

I agree to release and hold Kenwood Animal Hospital and Dog Latin Dog Training and Behavior Consulting, its subcontractors, the training facility, and their respective officers, directors, and employees harmless of any liability arising from this training activity and/or information or consultation provided by Dog Latin Dog Training and Behavior Consulting.
