    The information requested in this questionnaire is essential for our staff to provided focused and appropriate care from the first day of camp with us. Please be as honest and detailed as possible. Our staff would also be delighted to hear from you early in week if you feel that would be helpful.

  • The purpose of our Behaviour Code is to ensure that our campers know what is expected of them and are treated fairly by all adults working with them at ASK-All Special Kids’ events.

  • Basic principles

    ❖ Identify acceptable behaviour for children and young people

    ❖ Promote self-respect, self-control, self-confidence and co-operation

    ❖ Encourage children and young people to recognise and respect the rights of others

    ❖ Anticipate and resolve any conflicts that may arise and ensure that children and young people are aware of when sanctions will be put into place.

  • Examples of positive and negative behaviours (including but not limited to)
  • Positive: 

    Co-operative, Friendly, Respectful, Listening, Good manners, Joining in, Being Helpful, Rule-following.

    Negative:

    Bullying, Nasty comments, Shouting, Abusive Kicking, Cyber-bullying, Rude Hitting, Shoving, Biting.

  • ZERO TOLERANCE POLICY - If staff at ASK-All Special Kids become concerned that a child’s behaviour suggests either that he/she may be at risk of significant harm or that he/she may present a risk of significant harm to other children, ASK-All Special Kids child protection procedures will be followed. The parent/guardian will be informed of the situation and the camper may be asked to leave the Camp with immediate effect.

  • For the safety of the campers and our staff, we ask that you fill up this form as honestly as possible. We will not turn away any camper due to the information provided in this form. We will only use the information to provide the best nurturing environment for the campers.

  • Drop-Off/Pick-Up Procedures

    For Drop Off
    ASK Camp’s structured activities begin promptly at 9h. You are welcome to drop off your children anytime from 08h45-9h15.

    We would appreciate if you could try your best to arrive on time as late arrivals can be disruptive to your child’s day as well as to the other members of the camp group.

    You must bring your child to the sign in desk on the 1st floor and sign the sign-in sheet with one of our staff.

    We appreciate it if you try to avoid long goodbyes as this can be distressing for your child.


    For Pick-Up
    Your child must be signed out by you or the authorised person you have indicated. There is no exception to this rule so please be sure to fill in the “Pick-Up Authorisation” form, listing yourself and any other authorised persons for pick-up.

    Camp officially ends at 17h sharp. You are welcome to pick your child(ren) up between 16h45 and 17h00.

    In order to prevent distress for your child, it is very important you arrive on time. If
    you believe you may be running late, please call or text the assistant camp coordinator or head camp coordinator.

    Parents will be charged the following rates if they are running late:
    5-15 Minutes: 10 CHF
    16-30 Minutes: 25 CHF
    Over 30 Minutes late: 50 CHF
    Parents/Guardians will be billed and payment must be cleared before the child can return to camp.

    You will be contacted by the Administrator if payment is required.

  • Emergency Information

     

  • Permission to Provide Necessary Treatment or Emergency Care:

    I hereby give permission to the medical personnel selected by the camp director to order X-rays, routine tests or treatment; to release any records necessary for insurance purposes; and to provide or arrange any necessary related transportation for my child. In the event I cannot be reached in an emergency, I hereby give permission to the doctor selected by the camp director to secure and administer treatment, including hospitalisation, for the person named above. I recognize that the Camp Staff and Camp venue is not held liable for any accident or injury while participating in the program, and that in event of injury to myself and/or child, the Camp Staff and camp venue are not held responsible for reimbursement of medical expenses resulting from any injury. I will be responsible for the cost of all medical and dental treatment provided to my child while at camp.


    This completed form will be kept in the camp and may be photocopied for outings away from the camp venue. 

  • Permission slip for day outings & field trips

  • Usually (depending on the weather), we take the campers on a supervised outing or field trip on Tuesdays and Wednesdays.

    I (parent or guardian), hereby give permision for my child (name of camper) to participate in any of the weekly outings or field trips during camp hours. I understand that my child will be taking public transport to the destinations and will be engaging in various supervised activities with other camp members outside of the camp. I will provide a scanned copy of my child’s ID card and a travel pass (if applicable) to ensure that he or she will have it on hand each day in case of an outing.

  • Cancellation or Modification of Programmes

    ASK reserves the right to cancel or modify any program or programs because of an insufficient number of participants or for any other reason beyond its control. If this rare event occurs, ASK will offer the parents their choice of an alternative camp program or full refund of any monies paid.

    Usage of Images and Photos

    All photos, images, videos, audio recordings or interviews taken at or in connection with ASK events, seminars, camps or any other ASK children's programmes may be used as publicity on our website, brochures, newsletters, video commercials or any other recordings and written materials, provided that such use is specifically approved by his/her legal representative. In the event such use should be withdrawn after having been initially given, ASK reserves its right to seek indemnification for any damage thus incurred.
