    Welcome back to Financial Planning Ministry!

    Our mission is to help you maintain an estate plan that provides peace of mind while providing future funding for the kingdom work of our partner organizations (a complete list is in this form).

    Our continued document preparation assistance is available to those (“members”) who support our mission. We can update basic information in your estate plan. If your wishes fall outside the scope of our service, we will suggest other resources whenever possible.

    Please note that the scope of our services may have been redefined since you last worked with us. Your updated document(s) will contain the most recent template language that FPM provides.

    It is important to know that we are not a law firm and we don't give legal advice. The information we provide is for educational purposes only. The materials and forms we offer are not substitutes for advice or services from an attorney.

  • Before you Get Started

    • Review your documents and make a list of your changes in advance.

    • Articles II (people serving) and Article V or IX (beneficiaries) of your Trust contain the critical components that are typically changed. If you have amended your trust already, make sure you are looking at your most recent document set. 

    • Schedule B contains contact information for people and organizations.

    • Only complete this form once you are settled with your decisions and have all the information handy. All changes must be indicated in this form or they will not be implemented. 

    • Additional changes after submission may result in a request for a reasonable financial contribution to cover some costs of producing a replacement.

    • Expect to spend 15-30 minutes on this form.

    • This form is best completed in one sitting. If unable, click save to email yourself a resume link.

    • Proofread for accuracy (legal names, spelling, dates of birth, addresses).

    • Be prepared to upload copies of your signed trust and any subsequent amendments, when applicable. Handwritten changes on these copies will not be honored.

    If adding another child, please make sure to provide his/her legal name.
  • If you would like to include this child as a beneficiary in your estate plan, do not forget to add them to the estate distribution section later in this form.

  • Successor Trustee

    Find and review Article II of your current trust. At the death or incapacity of the last trustee (you or your spouse), the successor trustee will carry out the wishes and terms of your trust.
  • When updating your successor trustee(s)/executor(s), please re-list all of the people who will act in the order they will serve. We will update your Trust and Pour Over Will(s) accordingly.

    Find and review Article II of your current trust. Guardian(s) will care for your minor children if you are unable to do so. They will act as guardians only once they are appointed by a court. Monies from your estate will be available to the guardians for the care of your children until they are age 18.
  • When updating your minor child(ren)'s guardian(s), please re-list all of the people who will act in the order they will serve. We will update your Trust and any other affected documents.

    Find and review your Healthcare Directive. Your healthcare agent will make your medical decisions in the event that you become disabled and/or mentally incompetent.

    Please list the address(es) of each real property you still own as well as those you have acquired since receiving your last trust documents. Be sure to indicate if a property has already been titled to your trust.
  • If you selected 'yes' to the question above, follow these steps:

    • Locate your ownership deed, commonly named WARRANTY DEED, GRANT DEED, BARGAIN SALE or
      QUITCLAIM DEED (NOT your mortgage deed titled Deed of Trust or Reconveyance).
    • Locate your annual county Property Tax statement.
    • Write the street address at the top of each deed and tax statement

  • Charitable Gifts

    Find and review Article V or IX of your current trust. You are only looking at charitable gifts that come before the statement which reads 'all the rest, residue and remainder'. Changes to charitable gifts that fall after the 'rest, residue and remainder' statement will be addressed later in this form.

    If you didn't already select your own church as a partner organization, we encourage you to remember them now in your estate distribution. This is also where you can specify gifts to other organizations which are meaningful to you.
  • Gifts to Financial Planning Ministry

    You also have the option to support Financial Planning Ministry directly. Your gift will help us continue this ministry for you and others.
  • UP-FRONT GIFT: Gifts are typically one-tenth of one percent (.1%) of your estate value.
    For example: $100,000 to $400,000 estate = $400 gift; $1,500,000 estate = $1,500 gift.

    ESTATE GIFT: This gift will be given to Financial Planning Ministry at the time of your death. We recommend designating a percentage amount (5%, 10%, 15%...), as the value of your estate will change over time.

  • Important!

    1. If you need to remove, modify, or add gifts to charitable organizations listed before the 'rest, residue and remainder' statement in your distribution, please go back to the section titled 'Charitable Gifts'. Do not include those updates here (i.e. To add a 1% gift to your church, navigate to 'Charitable Gifts').

    EXAMPLE:

    2. If your current document has a charitable organization listed anywhere after the 'rest, residue and remainder' statement in your distribution (most often as an alternate beneficiary), you should make that change here.

    3. Distribution in this section must add up to 100%, excluding any percentage you have designated in 'Charitable Gifts' (i.e. 50% to John and 50% to David).


    4. If you are adding a new beneficiary, please specify a back-up/contingent beneficiary (i.e. Replace Joseph with John as 50% beneficary. Should John be unable to inherit, this share will go in equal shares to John's children").

  • Although this form is designed to give FPM information to update your Living Trust, we will also provide new copies of any other documents affected by your changes (Will, Healthcare Directive, etc.).

    Any new document we provide will need to be signed with a notary and/or witness(es). If the only change to a specific document is an address, you may save yourself that step and expense by updating the address(es) by hand.
