Per licensing rules, patient portal print-outs need to be validated by the doctor's office with a doctor's signature or stamp.

You can use this form to upload the following information:

• Vaccine Records
• Health Statement signed by the child's doctor
• Vision & Hearing Screening Records (4 & 5 year olds, only 2025 results are valid)

The health statement DOES NOT need to be on our form; many doctor's offices have their own form.  However, if you need one, you can access our copy here - Health Statement.

If your child has an allergy, an Allergy Action Plan is needed.  Both the child's doctor and a parent need to sign and date the form at the bottom.  We prefer to receive the original copy and will also need parents to provide all emergency medications in original packaging with prescription labels, if applicable.  Items should be turned in at the school office on your child's first day of attendance.  Action plans need to be renewed every 12 months, so it might be best to have your child's doctor complete it closer to the start of the school year.
