Each month, we feature a family or individual from the special needs community here in central Indiana. In order to be featured in the next available monthly magazine, please provide the following information and if applicable, a high resolution (300dpi) photo you would like to see on the cover and 5-8 high-resolution photos of you and your family for the feature article.

Use the questions to prompt your thoughts of what you would like to share. Not all of the questions will fit your story, but most will, so feel free to add to the information and make it your own.

This section is about you and your family's history and story.
What specific diagnosis or type of disability are you sharing about? How was this information obtained- birth, genetics, accident, by professionals, or research study? How has this impacted you or your family? What were the specific indicators that lead to this discovery? How would you explain to other families who know nothing about this diagnosis/disability?
What are your family members' names and children's ages? Where do your children go to school and does/do your special needs child/children attend school and/or receive other educational/behavioral services? If you have a special needs adult living at home, are they employed or attending a special services program?
Where do you live in Indiana? How long have you lived here for?
What have been some of the challenges you/your family have experienced, how have you overcome some of these struggles?
Feel free to use examples and stories that relate to you . You are welcome to share multiple examples.
Any advice you have for others who are experiencing similar situations and challenges?
What are some happy memories that really stay with you? Were there milestones or accomplishments that you/your child hit that you never thought would be possible?
What college/university did you/your Special Needs attend? How has school been challenging, what did you do in school to overcome some of these challenges?
What degrees or other professional certifications do you have?
What are your profession(s) and where do you work?
If you do not wish to include the name of the company you work for, simply list your profession. Has one or both parents had to adjust their professional life to accommodate your special needs child(ren)?
If you have children or a special needs adult still at home, with what activities are they involved?
Do they play a sport, participate in scouts or academic clubs? How have you adapted your family leisure time to meet the needs of both your special needs child(ren) and any other children? What have you had to improvise or give up?
Do you have any family traditions?
Do you and your family eat dinner together every Sunday or share traditional celebrations passed down from generation to generation? Do you have faith or cultural traditions that are special to your family? How have traditions, holidays, and special celebrations been adapted or modified so that they are inclusive for your special needs child(ren) and your other child(ren)?
