I, the undersigned, hereby request that the Office of the Registrar at Adams State University restrict the release of Directory Information contained in my student record. This means that no information, including Directory Information, can be released without my written authorization. This includes, but is not limited to, verification of degree(s) earned, attendance, or enrollment status. As the owner of my student record, I understand that I must submit a written request in order to rescind this restriction.
