In order to provide a safe and secure environment for our youth, we believe it is necessary to include the following questions as part of our application process. All information will be kept strictly confidential. (Police may access this information under warrant, if requested Answering “yes” to any of the questions may not necessarily preclude your involvement in ministry. A meeting will be arranged with a pastor so that you may discuss the circumstances. Thank you for your understanding.

Do you have any physical conditions that would prevent you from performing certain types of activities? If so, please explain.
MINISTRY AND MODELING
Much of what the disciples learned from Jesus probably came through their observations of what He said AND didn’t say; what He did AND didn’t do. Young people are very observant, and much of our ministry is unspoken. We are role models for them, and they tend to believe that what we do and say is biblical and Christ like. We have an obligation as leaders to live Godly lives and be “above reproach”. The following are standards to which we as a church expect youth leaders to adhere. Please check each item indicating that you have read and understand the expectations listed below. If you have any concerns or questions about the following, we can discuss it during your interview.
