Please use this form to let the school know:

that a student will be absent;

that a students will arrive late or leave early today;

why the student will be absent or late/leaving early;

when the student is expected to return to school;

Completing this form:

Please complete one form per child, as applicable.

If your child(ren) will be away for several days, you only need to complete this form ONCE until their return to school.

If your child(ren) will NOT return to school by the date you originally reported, please complete a NEW form.

This form is all you need:

Information from your completed form goes to the office.

Once this form is completed, you do NOT need to report the absence/late arrival to your principal or the office.
