Sweet Earline's Custom Cookie Order Form

PLEASE READ BEFORE SUBMITTING YOUR ORDER

We are a small team and each cookie is decorated by hand with care. To help us manage production and deliver the highest quality, please review the following before submitting your request.

Customers may selection one (1) cookie flavor per dozen ordered.
Cookie designs that require edible imagery (e.g.,logos, characters, personal pictures) are subject to a $5.00 up-charge per dozen.
Each order may include up to 6 unique designs.
A minimum of two (2) weeks' notice is required for orders under five dozen.
For larger orders (5 dozen or more), we recommend placing your order 3 - 4 weeks in advance to ensure availability.
Rush orders may be accepted depending on our production schedule and include a $20 rush order fee for requests made with less than two weeks' notice.
Orders are confirmed and added to our calendar upon payment. Orders are not considered final until payment has been received.
