Sweet Earline's Custom Cookie Order Form

PLEASE READ BEFORE SUBMITTING YOUR ORDER

We are a small team and each cookie is decorated by hand with care. To help us manage production and deliver the highest quality, please review the following before submitting your request:

Customers may select one (1) cookie flavor per dozen ordered.
Cookie designs that require edible imagery (logos, characters, personal photos) are subject to a $5.00 up-charge per dozen.
Each order may include up to six (6) unique designs.
A minimum of two (2) weeks’ notice is required for custom cookie orders.
This form is for orders up to six (6) dozen. For 7+ dozen, please complete our Large Order Request Form.
Rush orders may be accepted depending on our production schedule and include a $20 rush fee for requests submitted with less than two (2) weeks’ notice.
Orders are confirmed and added to our calendar once payment is received.

Submitting this form does not guarantee availability until your invoice is paid and your order is confirmed.

Design Expectations & Policies

Custom cookies are handcrafted and require artistic interpretation. While we make every effort to reflect the requested theme and overall aesthetic, exact replication or inspiration images is not guaranteed.

Clients are strongly encouraged to provide multiple inspiration images that clearly reflect their desired color palette and design style. If limited or conflicting references are provided, Sweet Earline's will interpret the design using professional judgement based on the information available at the time of approval.

Certain techniques such as stenciling, metallic finishes, and hand lettering are completed by hand and may contain slight variations. These characteristics are inherent to custom decorated cookies and are not considered defects.

Standard Orders (1-6 dozen) are non-refundable once production begins due to the custom nature of decorated cookies and scheduling commitments.

Clients are responsible for reviewing all design details on their invoice carefully before payment. Once an order has been completed and delivered or picked up, design changes or adjustments cannot be made.

Pick-up, Courier & Shipping Information

Local Pickup: Orders are available for pickup in Rockville, MD during designated pickup hours. A reminder email will be sent 24 hours prior to your scheduled pickup date.
Courier Delivery: Customers are responsible for arranging courier pickup and payment and providing driver details prior to the scheduled pickup time. Sweet Earline’s is not liable for courier delays, damages, or mishandling once the order leaves our possession.
UPS Shipping: Available for select cookie orders. Sweet Earline’s is not liable for carrier delays or damages once the order has shipped. Tracking information will be provided upon shipment.

Edible Imagery Add-On (Optional)

Edible Imagery includes logos, characters, and printed photos. This add-on is charged per dozen and will be included on your Square invoice.

Design Description/Custom Requests Details

Please describe your desired cookie design as clearly as possible. You may include: • Theme, event type, or occasion (e.g., baby shower, birthday, wedding) • Preferred color palette or style (e.g., pastels, neutrals, metallics) • Text or names to include (if any) • Specific shapes or elements you’d like featured.

Packaging (Included)
All Sweet Earline’s cookies are individually wrapped in clear, air-tight cellophane bags to maintain freshness. This is included in standard pricing.

Additional packaging décor (such as bows, custom tags, favor, or gift box presentation) is available for an additional fee.
