Liability Waiver & Participation Agreement

As a student of Unity Dance & Movement, I am aware I am engaging in dance instruction at my own risk. I agree that I will not hold the studio, staff, or instructors liable for any injuries sustained during dance instruction. If I have any particular injuries or concerns, I will disclose this concern with the instructor prior to class. I will ensure that I exercise caution in all movements.

I understand that as a dancer, I am not permitted to illegally share, or record instruction without the express permission from Unity Dance & Movement and Jenifer Dobbins.
