AP17 - Change of Details Form

* If you wish to change your forename or surname you will need to upload proof of your name change, ie marriage certificate or deed poll.
Please note, if you also wish to change your 'published' contact details, ie your details as they will appear on the Central/EST Registers:
- Log on to Access Elmhurst
- Click on the My Details tab and then the Published Details grey button
- Click Save once updated
Your new details will be updated on the Central/EST Registers within a couple of hours of you making the changes.
