    Used to update a school order when taxes need to be removed.

  • **Check CRM in the shipping notifications to see if there is a certification on file. If EXPIRING within 3 months, attach completed tax exemption certificate below. ALL NEW REQUESTS MUST HAVE FORM ATTACHED.**

  • If the Exemption Certificate expires within the next 3 months, contact the customer to  obtain an updated certificate showing tax status.

  • IF THE EXEMPTION FORM IS NOT COMPLETE, REACH OUT TO THE CUSTOMER TO COMPLETE AND THEN RESUBMIT.

  • Certification must include:

    • Chef Works Name and Address
    • "Uniform" as what is being resold
    • Date

    If these are not inlcuded, the form will be rejected.

  • Upload PO to WMS and update PO field in Order Entry before releasing the order.

    • To be completed by Credit Applications 
    • Should be Empty:
