If you previously completed (prior to 2021) our Immunization Administration Training for Pharmacists and are looking to obtain a duplicate copy of your certificate, please fill out the following form.

Pharmacy Technicians: 
If you are a pharmacy technician and completed our Immunization Administration Training for Pharmacy Technicians course, please do not fill out this form. Your certificate of completion will be sent to you via email 7-10 business days following completion of the course.

If possible, locate your documentation from ACPE for completion of the course prior to submitting this form. Confirm that the provider identification number for the course is 0107. The provider identification number is the first four digits of the ACPE UAN (Universal Activity Number). For example, for the UAN 0107-0000-20-364-H06-P, 0107 is the provider identification number.

If the provider number on the course you completed is something other than 0107, then CEimpact was not the provider for your training and another provider should be contacted. You may search for providers based on their provider number here.
