Ms. Lois Stacks had a passion for controlling the unwanted pet population in Lake County, FL so she generously bequeathed funds to help defray the cost of spaying and neutering cats and dogs belonging to residents of that area. Established in 1995, The Stacks Foundation strives to carry out the desires of Ms. Lois Stacks by providing financial assistance for spay and neuter procedures to residents of Lake County, FL. Please help fulfill Lois Stacks' vision by getting your pet spayed or neutered! Thank you!

Who is eligible?
Any resident of Lake County, FL! Certificates provided by The Stacks Foundation, Inc. are intended to help those on a limited income with the greatest need. If your annual household income is over $75,000, we encourage you to refrain from applying for a rebate to save the funds for those less fortunate families. Thank you.

Any stipulations?

• You must be a Lake County resident. Proof of residence will be required on the rebate application.
• Rebates are ONLY valid to help offset the cost of spay and neuter procedures. They cannot be used towards the cost of vaccinations.

To apply for a rebate:
Please follow all directions carefully. If your request form is filled out incorrectly or is missing any pertinent information, it will immediately be discarded.

1. Fill out a Rebate Request Form at our website link. Complete the form then click “submit” at the bottom of the application. You will need the following items to complete the request:
   1. Your full name
   2. Your physical address
   3. Your mailing address
   4. Pet information:
      • Name
      • Cat or Dog
      • Male or Female
      • If dog, over or under 40 pounds
   5. A proof of residence. To be valid, this document must contain the following printed information:
      • Your full name
      • Your physical address (that matches application)
      • Issue date within 60 days of your application
   6. A copy of your invoice showing each pet's spay or neuter surgery has been completed
      • Rebate applications must be received within 60 days of each pet's surgery
      • Invoice must show:
        • Your name
        • Your address
        • Your pet's name
        • Date
   7. There will be a small $3 processing fee to be paid at the time of application submission.
2. Please allow 6-8 weeks for processing of your rebate check.

As outlined on our Facebook page, a proof of residence is required on this form and must be dated within 60 days of your application. Examples of these include electric bill, water bill, phone bill, cable bill, etc. These can be attached as a PDF, word document, or picture file. Driver's licenses are NOT ACCEPTED.

Please ensure your application is filled out correctly, and your proof of residence is valid according to the descriptions above. Otherwise, we will not be able to process your request.
