By signing this contract, enrolling online and/or attending classes, events, activities and other programs or using any equipment offered by F45 Training Mountain View South (“F45”), I hereby acknowledge and agree on behalf of myself, my heirs, personal representatives and/or assigns (collectively “me” and/or “myself”), that exercising and using F45 facilities either inside or outside the studio, may involve a significant risk of physical harm or personal injury, including permanent disability and/or death. I understand that classes offered by F45 may be physically strenuous and I assume full responsibility for any and all injuries or damage which are sustained or aggravated in relation to the sessions and/or facilities or any area where the workout takes place, including on ashphalt, grass, or online at home. I have been fully informed of the strenuous nature of the program and I assume all risk for my health and well-being. I hereby affirm, and I re-affirm each time I take part in a workout, that:

I am in good physical condition and do not suffer from any known disability or condition that would prevent or limit my participation in the workouts; and
I am participating in the workouts voluntarily and at my own risk.

I further acknowledge that F45 including its officers, employees and agents shall not be liable to any person whether in contract, tort, under statute or otherwise for any injury, loss, damage, death or economic loss whatsoever, whether consequential, direct or indirect, caused by or connected with my participation in F45 classes.

If I am enrolling a minor (16-18), the above release applies equally to their participation.
