2025/2026 - Enrolment Ballot Notification Form

Use this form to notify the name of a child you would like included in our next Enrolment Ballot for students residing outside the Cotswold Mātāhae School Enrolment Zone. When you click on the submit button at the bottom of the form, you will receive a copy of the completed form by email to the email address you have provided on the form. This is confirmation that we have received the form and will include this child in our next ballot.

Applications for enrolments will be processed in the following order of priority:

First priority is not applicable at this school because the school does not run a special
programme approved by the Secretary for Education.
Second priority must be given to any applicant who is the sibling of a current student of the
school.
Third priority must be given to any student who is the sibling of a former student of the
school.
Fourth priority must be given to any applicant who is the child of a former student of the
school.
Fifth priority must be given to any applicant who is a child of an employee of the school’s
board or a child of a member of the school’s board.
Sixth priority must be given to all other applicants.

All Christchurch primary schools operating an enrolment zone are required to use the same balloting dates.
