I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that the Canandaigua Knights Youth Hockey Conference have put in place preventative measures to reduce the spread of the Coronavirus/COVID-19 in events and/or activities organized by the Canandaigua Knights Youth Hockey Conference.
I further acknowledge that the Canandaigua Knights Youth Hockey Conference cannot guarantee that I, or any of my immediate family members, will not become infected with the Coronavirus/Covid-19 if I, or my immediate family members, attend and/or participate in events and/or activities organized by the Canandaigua Knights Youth Hockey Conference.
I further acknowledge that the Canandaigua Knights Youth Hockey Conference does not, in any way, own, control, or manage the Greater Canandaigua Civic Center, or any other facility that may be used to host events and/or activities organized by the Canandaigua Knights Youth Hockey Conference.
I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, inactions, omissions, or negligence of myself, my immediate family members, and others, including, but not limited to: Canandaigua Knights Youth Hockey Conference volunteers, directors, and members (including, but not limited to players and their immediate family members); and the employees and/or other guests of the facilities being used by the Canandaigua Knights Youth Hockey Conference.
I voluntarily seek the services provided by the Canandaigua Knights Youth Conference for myself and/or my family members and I acknowledge that by attending and/or participating in events and/or activities organized by the Canandaigua Knights Youth Hockey Conference that I am increasing the risk of exposure to the Coronavirus/COVID-19 for myself and/or my immediate family members. I acknowledge that I and any other participating family members will comply with all set procedures to help reduce the spread of Coronavirus/Covid-19 while attending any events and/or activities organized by the Canandaigua Knights Youth Hockey Conference.
Furthermore, my, or my immediate family members', attendance at ANY events and/or activities organized by the Canandaigua Knights Youth Hockey Conference is an attestation that:
* Neither I, nor any of my immediate family members, are experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* Neither I, nor any of my immediate family members, have traveled internationally within the last 14 days.
* Neither I, nor any of my immediate family members, have traveled to a highly impacted area within the United States of America in the last 14 days.
* Neither I, nor any of my immediate family members, believe that I/we have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* Neither I, nor any of my immediate family members, have been diagnosed with Coronavirus/Covid-19 and are not yet cleared as "non contagious" by state and/or local public health authorities.
* I, and my immediate family members, are following all CDC recommended guidelines as much as possible and limiting our exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold Canandaigua Youth Hockey Conference harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or my immediate family members and/or property that may be caused by any act, or failure to act of the Canandaigua Knights Youth Hockey Conference, or that may otherwise arise in any way in connection with any services received from the Canandaigua Knights Youth Hockey Conference, its volunteers, directors, and/or its members. I understand that this release discharges Canandaigua Knights Youth Hockey Conference, its volunteers, directors, and/or its members from any liability or claim that I, my immediate family members, my heirs, or any personal representatives may have against the Canandaigua Knights Youth Hockey Conference with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Canandaigua Knights Youth Hockey Conference, its volunteers, directors, and/or its members. This liability waiver and release extends to the Canandaigua Knights Youth Hockey Conference together with all volunteers, directors, and members.
