    Communities In Schools (CIS) of High Point is sponsoring a new Leadership & Ambassadors Training (LAT) program for middle and high school students where the focus will be on engaging participants in leadership development, peer-to-peer mentoring, critical discussion on pressing issues, and training in essential life skills.


    Your student has been nominated to participate in the inaugural session of this program, set to take place during the 2020-21 school year. We hope that you are able to support your student’s participation in this unique opportunity. Applications should be completed and submitted by September 30, 2020. 

    Our mission is to surround students with a community of support, empowering them to stay in school and achieve in life.

    Program Details:

    • Ten (10) students will be selected from each school.
    • All training sessions will take place at the Communities In Schools main office:
      • 149 Church Avenue, High Point, NC 27262
    • CIS staff will handle all coordination and communication for the program, including providing snacks and transportation for students who need it utilizing a contracted transportation company. Please inform staff if your student will need transportation to and from sessions.
    • The proper procedures for COVID mitigation will be in place:
      • Temperature checks will be done for all individuals entering the building
      • Face masks are required to be worn, covering nose and mouth
      • Participants will maintain 6 feet of distance
      • Regular application of sanitizer and hand washing
      • All surfaces will be cleaned regularly
      • Individuals who are exhibiting symptoms are asked to notify their respective coordinator and make plans to remain at home for the suggested period for quarantine or until deemed appropriate by medical authorities, doctor, etc.
    • There will be a virtual information meeting for parents/guardians and students held on Wednesday, September 23, 2020 at 6:00 p.m. Please feel free to attend and meet CIS staff who will be available to answer any questions or concerns regarding the program.

    • Training Session Schedule:
      • Ferndale Middle – Tuesday following the 1st Monday of each month, 4:00-6:30 p.m.
        • 1st session will be on October 6th
      • HP Central – Tuesday following the 2nd Monday of each month, 4:00-6:30 p.m.
        • 1st session will be on October 13th
      • Welborn Academy – Tuesday following the 3rd Monday of each month, 4:00-6:30 p.m.
        • 1st session will be on October 20th
      • T.W. Andrews – Tuesday following the 4th Monday of each month, 4:00-6:30 p.m.
        • 1st session on October 27th
      • Southwest Middle - Thursday following the 3rd Monday of each month, 4:00-6:30 p.m. 
        • 1st session will be on October 22nd 
      • Southwest High - Thursday following the 4th Monday of each month, 4:00-6:30 p.m.
        • 1st session will be on October 29th
    • This program will be led and facilitated by Greg Commander — CEO of Commander Peace Academy and nationally renowned speaker on life skills and anti-violence initiatives. In addition to the in-person training sessions, Mr. Commander will remain in contact with your student throughout the month through mentoring outreach.

    By completing and signing this form, students are agreeing to adhere to all program rules, guidelines, and procedures, and will remain committed to participating in the Leadership & Ambassadors Training (LAT) program until the end of the 2020-2021 school year.

  • Parental Consent

    Please complete the section below to permit your student’s participation in the Leadership & Ambassadors Training (LAT) program and affirm your agreement with all details stated in the program information packet.
  • Completing this consent form gives permission for your child’s participation in the following activities:

    1. Release of confidential information to CIS staff, affiliated personnel, and the Department of Health & Human Services (i.e. access, track, and report grades test scores, etc. to determine additional services needed.)
    2. Referrals to agencies for specific services.
    3. Media coverage of CIS programs (i.e. interviews, photos, television, etc.)
    4. Interviews, tests, and questionnaires for program evaluation.
    5. Participation in a one-on-one volunteer relationship (i.e. tutoring, mentoring, etc.)

    Furthermore, completing this consent form signifies your commitment, as a parent/guardian, to:

    • Participate in parent-staff conferences to discuss your child’s progress, either through home visits, virtual meetings, or phone calls.
    • Not hold Communities In Schools of High Point, Inc., CIS employees, Guilford County Schools, CIS volunteers, or any school staff liable for dental, medical, and/or surgical treatment in such cases of illness, accident, or any emergency situation that should occur in connection with authorized activities or travel.
    • Read and review this entire application, agree to its terms & conditions, and submit this application by September 30, 2020.
