I assume full responsibility and agree to hereby release Jason Miller, Victory Sports, Greyfriars Classical Academy, and its agents, employees, counselors, volunteers, coaches, and any facilities they use, from liability on account of injuries sustained while participating in these activities. I give permission to be medically treated for illness occurring or injury sustained during such participation. I certify that I am covered by medical insurance which will reimburse Jason Miller, Victory Sports, and/or Greyfriars Classical Academy for expenses incurred by them, their agents, employees, coaches, counselors, and volunteers on account of medical treatment ordered at their discretion and also to indemnify them for any expenses not reimbursed by such insurance.
