  •  For US-based organizations interested in paying with a Purchase Order:

    Please fill out this form at least one week prior to the Program Start Date. You may enter up to 9 participants on a single form. Please use the "additional comments" if needed.

    Please have your signed PO ready to attach to the end of the form.

    To complete this form, you will need:

    • Participant name(s), email address(es), and professional role(s)
    • Billing contact information
    • A signed Purchase Order with complete billing contact information,  payable to  MINDFUL SCHOOLS: 1260 45th Street, Emeryville, CA 94608

    We will review your PO and confirm registration within 3 business days. Our Accounting Team will follow up to complete the payment process.

    Please be sure to complete this form at least one week before the Program Start Date.
