MEDICAL WAIVER: I understand that there are risks of physical injury associated with, arising out of and inherent to these activities. As parent/guardian of the child/children listed above, I give Spero Program for the Performing Arts permission to seek medical attention for my child in case of accident or emergency. Off Campus Lunch Permission
I give the following students permission to walk off of the Hope International Campus for lunch breaks and acknowledge that there may not be adult supervision. I release Spero Program for the Performing Arts of any liability if and when the following students leave the Hope International Campus.

Commitment Agreement
I agree to assist my child in following through on their commitment to this production. I also agree to volunteer as per the parent volunteer commitment agreement, and to sell at least 10 tickets to friends and family. I understand there is a $200 production fee for my child to participate, and that there are payment plans available. I commit to be in attendance at all scheduled rehearsals and ALL tech/dress rehearsals and performances for the musical production.

I understand that absences are not allowed unless pre-approved in an agreement between myself and the directors. Exceptions will be made, to a reasonable degree, for illness or emergency situations. I also understand that excessive absences may result in my being asked not to participate in the production. I will be on time to all rehearsals and performances.
